RE: Rec/Sports Camp 2015

Dear Parent:

The next three pages are the forms that must be submitted to our offices in order for your child(ren) to participate in the camp this year.  These are very easy to fill out and submit in this new format.  We opted for this manner of submission to contribute to the environment and diminish the use of paper forms. 

Please complete all required information and, when prompted, include the names and email addresses of the three emergency contacts.  The form will be forwarded to these people in the order they’re entered into the form.  When the first signature is completed, the form will automatically be sent to the second contact.  The same happens regarding the third signature.  When all signatures are received, your document will be marked as completed.

Robbinsville Recreation will not use the names and email address entered into the forms for any other purposes other than matters related to camp.
